  • Do I need a referral from my child's doctor?

    Yes! We ask that each of our patients has a referral sent from their physician prior to scheduling an occupational therapy evaluation.

  • Do you take insurance?

    We are in network with Aetna, Blue Cross Blue Shield, Cigna, Tricare, and UnitedHealthcare. We do not currently accept any state funded health insurance, such as Meridian or Molina, as well as any managed care plans. If we do not accept your insurance, we do have private payment options. If you have questions about your specific plan, please call our office.

  • What can I expect during the first visit?

    When you arrive for your child’s occupational therapy evaluation, our office staff will greet you to check in your child. Your child’s occupational therapist will ask you a series of questions about your child’s developmental and medical history. The occupational therapist will also perform standardized assessments with your child to determine if they qualify for our services. We recommend planning approximately 90 minutes for their initial evaluation.

  • What should I bring with me during my child's evaluation?

    Please bring your driver’s license, insurance card, and preferred method of payment for possible out of pocket expenses or co-payments.

  • Do I need to fill out paperwork before my child's evaluation?

    Yes! After scheduling your child’s appointment, our office staff will email you online forms to complete. Please complete these prior to your child’s evaluation in order to help your child’s therapist best plan for the evaluation.

  • Do you offer any other services at your clinic?

    AppleTree Therapy offers only occupational therapy services. We do, however, share a building and waiting room with ABC Pediatric Therapy, which provides both speech and physical therapy services.
