Dear Parents,

This page contains the paperwork necessary for completing an evaluation at AppleTree Therapy. We typically begin therapy with an initial evaluation. This usually takes about 90 minutes. If your child qualifies for occupational therapy, appointments are scheduled on a regular weekly basis, one hour sessions, same time each week and are confirmed for as long as your child requires therapy.

To schedule an appointment for an evaluation at AppleTree Therapy, please call 618-690-0068 to provide us with initial contact information and to speak with us regarding your child’s medical history, your main concerns and your child’s specific needs for therapy. We will do our best to answer any questions and appreciate an opportunity to speak with you directly about our services, scheduling, and working with insurance. We will schedule the evaluation on that day.

After school time slots are typically most popular and we often have a wait list for them. Since appointments remain filled for extended periods of time, it is frequently necessary for children to be placed on a wait list prior to being scheduled for an ongoing after school appointment time. Waiting times vary – based on the time of year and other referrals received.

To conduct the evaluation we need a Doctor's prescription either faxed directly from the Pediatrician's office to 1-888-452-2930 or you can bring the original prescription to your first appointment. Also, please bring your insurance card.
